AFRALTI takes the issue of data protection very seriously and would never knowingly use an image of you without your prior consent.  We would therefore ask you to read through the information below.  Please fill in the details in the white spaces on the table and then sign and return the form to us.

I understand that this image will only be used for the following purposes:

• AFRALTI’s electronic (including website, social media) and printed information, displays and exhibitions relating to the activity shown in the picture.
• Any similar AFRALTI campaign or related area.
